We want everyone at our group to feel safe, happy and respected.

Before you fill in the membership form, please read and agree to the rules below.

Be Kind and Respectful

Please treat everyone with kindness and respect.

Do not bully, threaten, shout at or upset other people.

Please respect other people's personal space and belongings.

Listen to Staff and Volunteers

Please follow reasonable instructions from the group leader and volunteers.

These instructions are there to help keep everyone safe.

Use Art Materials Safely

Please use art materials, equipment and other items safely.

Do not deliberately damage equipment, furniture or another person's belongings.

Tell Us If You Cannot Come

If you normally attend the group but cannot come to a session, please let us know if possible.

This helps us know who to expect.

Support Workers and Carers

If you need a support worker or carer, they must stay with you during the session and give you the support you need.

Chance to Choose volunteers do not provide one-to-one care or personal support.

Our volunteers cannot replace your support worker or carer.

Photos and Videos

We may sometimes take photographs or videos at Chance to Choose.

You will be able to tell us on the membership form whether you give permission for photographs or videos of you to be used.

You can say Yes or No.

Keeping Everyone Safe

If there is a problem, please speak to the group leader or a volunteer.

We will listen and try to help.

Serious or repeated behaviour that breaks these rules may mean that you are asked to leave the session or cannot attend the group for a period of time.

This is to help keep everyone safe.

Keeping Your Information Private

We will keep the information you give us on your membership form private and secure. Only authorised people who need the information to help support you or keep you safe will be allowed to see it. In an emergency, we may share relevant information with emergency services or other people who need it to help you.

• You agree to follow the rules while attending Chance to Choose.

• You understand that you will need to complete a membership form before joining the group.
